Depressing Figures for Acomplia weight loss pill

I worried about taking a new drug to market that had a completely new central nervous system mechanism: Acomplia (rimonabant). CNS makes me nervous. I used to work in the area, and I have a healthy respect for how little we know about it. So when you come in with something new, you have to be worried about what's going to happen, and whether your clinical trials are going to be enough to tell you about it.

And sure enough, the long, long delay at the FDA for the drug, which was (in theory) supposed to be approved in the first half of 2006, turned out to hinge on CNS side effects, among them "suicidal ideation". Now a meta-analysis has come out in The Lancet which suggests that patients taking the drug in Europe (one of the few places you can take it) have a much higher risk of depression.

You have to be careful with meta-analyses. But this one's noteworthy because, as the authors point out, depressed mood was an exclusionary factor for the studies concerned. Yet even after winnowing out those patients, the study patients seem to have been 2.5 times as likely to drop out of the trials due to depression as compared to the placebo groups. The studies totaled 2503 patients on the drug, and 1602 in the placebo groups. Depression showed up in 74 and 22 cases in those groups, respectively, which does seem to be a real effect, especially when you start by excluding anyone who seems depressed.

Compare that with the Avandia meta-analysis that has made much so much news (and come close to sinking the drug completely). Out of 14,000 patients, that one had 86 cardiac events in the treatment groups and 72 in the controls, and this in a population with underlying cardiovascular trouble. Depression is not as serious an outcome as a heart attack, to be sure, but it's nothing you'd sign up for, either. Sanofi-Aventis should stop being upset that they haven't gotten the drug on the market here, and start being glad that the lawyers here didn't get a chance to strip a few billion dollars off of them.

Sanofi Says No Plans to Try to Sell Acomplia in U.S. for Several Years

Sanofi-Aventis will not try again to sell diet drug rimonabant (Acomplia / Zimulti) in the United States for another three or four years, the company said on Sept. 17th, disappointing millions of dieters and leaving the door wide open for competitors.

Merck and Pfizer both have drugs that like rimonabant target the CB-1 receptor in late-stage trials, and a number of other companies are developing similar drugs as well. While sales of rimonabant in the year it has been on the market in Europe have been disappointing, Sanofi made it clear it plans to forge ahead with studies of Acomplia in hopes of ultimately getting it approved in the U.S. both as a diet drug and treatment for type 2 diabetes. http://www.freewebs.com/acomplia

"I'm confident in Acomplia because it is a unique product," Sanofi's head of research Marc Cluzel told reporters and analysts at a presentation on the company's research and development program. While Acomplia was launched in Europe in 2006 and is also available in a handful of other countries, Sanofi withdrew its application in the United States after an FDA advisory panel unanimously recommended against approving the drug out of concern over side-effects including depression and suicidality.

Cluzel said Sanofi expects to refile its application with the FDA after longer-term data on the use of Acomplia has been gathered from the large CRESCENDO trial, which got underway in December 2005. Sanof said it has enrolled approximately 11,000 of the 17,000 participants planned for this trial, and indicated it expects to have data from this study available to share with the FDA in 2010.

But while Sanofi's news on the near-term future of Acomplia as a diet drug may have been disappointing, the company indicated it now plans to press ahead aggressively with new studies aimed at winning approval of rimonabant as a main-line treatment of type 2 diabetes.

Sanofi said it plans in the first quarter of 2008 to launch a head-to-head trial comparing Acomplia to Merck's diabetes drug Januvia.

The company said it hopes to file for regulatory approvals worldwide for Acomplia as a treatment for diabetes in 2009, and follow that up by filing for a drug that is a combination of Acomplia and the older diabetes drug metformin in 2010. Sanofi said it also hopes to file for regulatory approval of a drug that is a combination of Acomplia and a cholesterol-lowering statin in 2011.

Shares in Sanofi-Aventis slipped back in morning deals amid concerns over the pharmaceutical group's drugs pipeline and notably the announced four-year delay to US approval of Acomplia, traders said. Sanofi shares were trading down 0.92 eur or 1.49 pct at 60.65, while the CAC-40 was up 9.75 points or 0.17 pct at 5,698.51.
